you in the unlikely event that your passport is lost or stolen
while abroad.

In tandem with your medical form, this document provides us with
proof of your coverage.

You must photocopy and submit a copy of your appropriate visas to
our office. We use this to help you in the unlikely event that your
passport is lost or stolen while abroad.
