Medical history and pertinent information filled out by you and your physician.  Please note that your physician will need to complete and sign the last page of this form.  Without a doctor's signature you will not be cleared to participate in a program abroad.

Visiting students use this form to provide contact information for your family or friends in case of an emergency abroad. St. John’s student should visit St. John’s Central, and update their emergency contact information in UIS.

Photocopy of Passport
Due Date: December 5, 2008
Photocopy of the picture page of your passport. We use this to help you in the unlikely event that your passport is lost or stolen while abroad.

Photocopy of Insurance Card

You must be covered by international health insurance while abroad. In tandem with your medical form, this document provides us with proof of your coverage.

Photocopy of Visa
Due Date: December 5, 2008
You must photocopy and submit a copy of your appropriate visas to our office. We use this to help you in the unlikely event that your passport is lost or stolen while abroad.
