any individual involved in an emergency situation requiring medical assistance should notify the Health Service Office directly or notify:

supplying as much information as possible about the situation.

Upon notification of an emergency the nurse will determine from available information if it appears necessary to call an ambulance. If an ambulance is to be called, Public Safety will be notified and will make the request.

If able, the individual will be transported (or walk) to the Health Service Office for further evaluation and/or care.

If the student needs to go to the hospital (as determined by the nursing staff) but does not require an ambulance and has no other mode of transportation, Public Safety will be notified. Transportation may be arranged at the discretion of the Director or his designee depending on the nature of the illness or injury. Transportation by Public Safety to a hospital should be treated as a request of 'last resort'.
In all cases, as appropriate, family will be notified.

Documentation is recorded on student/employee medical records in the Health Service Office and the incident is logged by Public Safety.

If the Health Service Office is not open, Public Safety will handle the situation.
