Students should make a copy of all completed documentation
before submitting it to Student Health
Services. Please include your X number
on all submitted forms. Once health records are
submitted to the office, students requiring a copy must provide
written authorization and a $5 fee payable to St. John’s University
for each copy requested. Students must allow adequate time for
their request to be processed.
New York State Law mandates that Immunization and Meningitis
forms for all new students be submitted to Student Health
Services.
Forms are due by:
- December 15, 2011 for the Spring
semester
- May 15, 2012 for the Summer
and Fall sessions
These 3 forms must be
submitted by new students in the first semester of
their enrollment. Continuing students do NOT need to resubmit
the forms.
